How-to Guide for CRNAs Who Have Graduated from New York State Nurse Anesthesia Programs Since 2005.  

Before beginning, you can click here for the NYS Board for Nursing website to find the requirements and forms.

1. Did you graduate from a New York State Nurse Anesthesia program in 2005 or later? if yes, proceed to number 2.
2. Go to www.nysed.gov
3. Certification and Licensing
4. Professionals
5. Nursing
6. Application Forms: Nurse Practitioner
7. Complete Form 1... #10: check box marked "Acute Care "; #11: check box marked "B"
8. Complete Form 2... #9: check box marked "Acute Care"

Once submitted, call State Ed to confirm they have received the application and it is under review. If any further information is needed, they will contact you.
